Summary of Services:     Community Health Care provides primary medical and dental care for individuals, families, and children.  Routine checks and screens, including well-woman exams are available, as well as services for smoking cessation and chronic disease management. 

Schedule:                       Monday – Friday:  8:00 a.m. – 5:00 p.m.

Cost:                               Harrisonburg Community Health Center provides services on a sliding fee scale.  To be eligible for discounted services, clients must apply by filling out the “340B Prescription Discount Program Plan/Sliding Fee Application” located in both English and Spanish on the CHC website.
